My condolences go out to his family.
Really disheartening to see something like this relegated to areas for secondary news reports and no attempt made to verify facts in the reports.

the death of this officer is a result of complacence and untrained personal in an unfamiliar environment compounded with lack of S.O.P, equipment and resources for a successful mission....
the procedure for a field raid is...
1. the army will do a recon of the area for 1 week prior to raid
2. on the morning of the exercise the army will do a 360 sweep with a heavy machine gun and grenades to remove any traps or combatants.
3. they will secure the perimeter and provide tactical/fire support for the police.
then the police will come in and do their work.
From the pics I can tell the officer has no bush patrol experience just by how his weapon is being carried.
