It's a known problem with the older model mazda b2500 / ford ranger.. I didn't expect to see this popping up on the newer models..
We had almost identical symptoms to yours and they actually went away after some minor servicing.
There was a coolant leak on the lower radiator hose where it connects to the block, so that accounted for the coolant loss.
The glow plugs were bad, so that accounted for the rough idle when cold. Also the engine idle speed was set a little too slow.
I'd check those 1st and my fingers crossed it's that simple a fix..
