Copies and originals of affected children and parent's computer generated birth certificate AND machine readable passport. Non-MR (machine readable) documents would NOT be accepted.
Copies and originals of child/children last report book/school-based assessment
SEA students can walk with their registration form (for their new school)
Parents would be required to bring 3 (three) utility bills and/or proof of payment (electricity, water, phone). If you are renting, please ask your landlord to provide copies of such.
Parents also required to bring land/home ownership certificates or in the case of rent, proof of payment of most recent payment.
A 3000 word letter is also required detailing the events that took place.
Any supporting documents such as :- digital photos of mud stained/water damaged books/clothes. Receipts from book stores etc.
Ensure you have ALL these documents and ask for the Relief Officer at the Ministry
Once approved, you can collect your cheque in 10 working days at the Ministry's Accounts Department.
