Adam and Eve's son (Cain) married his sister, Luluwa. This is found in the Second Book of Adam and Eve. Their genetic make up wa s much different from ours so there was not the deformity / mental illness that would manifest today for the same reason.

As for the first point, I wouldn't really blame Adam. If someone told me they created me out of endless love but if I ate an apple everyone to ever exist from this point on will have to die... I would think they were crazy.
