Durability of BNT162b2 vaccine against hospital and emergency department admissions due to the omicron and delta variants in a large health system in the USA: a test-negative case–control study

Findings

Analyses were done for 11 123 hospital or emergency department admissions. In adjusted analyses, effectiveness of two doses of the BNT162b2 vaccine against the omicron variant was 41% (95% CI 21–55) against hospital admission and 31% (16–43) against emergency department admission at 9 months or longer after the second dose.

Through normsl regulatory process, or still under emergency use?
By now, there should be no pressure to get them pushed via emergency use route...

Researchers in South Africa tested the ability of antibodies in blood to disable BA.4 and BA.5 viruses in a lab. In people who were unvaccinated but recently recovered from a BA.1 infection, they saw a more than seven-fold drop in the ability of their antibodies to neutralize BA.4 and BA.5 viruses. In people who'd been vaccinated but recently had a breakthrough infection caused by BA.1, the drops were smaller, about three-fold lower.

By way of comparison, the World Health Organization uses an eight-fold drop in neutralization as the threshold for the loss of protection that requires an update to seasonal influenza vaccines.

The study results led the researchers to write that "BA.4 and BA.5 have potential to result in a new infection wave," making Covid-19 vaccinations and booster shots crucial to stopping the next wave.

"Our conclusions from this are, first, that Omicron by itself is not a great vaccine, right?" said Alex Sigal, a virologist at the Africa Health Research Institute who led the study. "Just because you were infected does not mean you have a lot of protection from what's coming next."

Dr. Eric Topol, a cardiologist who is the founder and director of the Scripps Research Translational Institute, praised the research, pointing out that this lab was also the first to characterize the first Omicron variant: "They've been first-rate all the way through the pandemic."

He said that overall, the finding was not good news. Even people who recovered from a Covid-19 infection as recently as December or January can be reinfected by these new subvariants.

but even if u are reinfected, previous infection still protects against severity, not so?

Therefore the BA1-specific vaccine boosters in trials now might already be outdated lol

Interesting write-up of a biorxiv preprint re Omicron sublineage immune evasion. Three key points:
-the BA2.12/BA4/BA5 variants show higher immune evasion to specifically BA1+vaccinated plasma
-consequently BA1-targeting boosters could actually be a poor choice for getting broader protection
-Bebtelovimab is still working against all extant variants

Results
The study results showed that the Omicron BA.2 sublineages, such as BA.2.13 and BA.2.12.1, had higher ACE2-binding affinities than BA.1, but BA.5/BA.4 has the poorest receptor-binding ability due to R493Q and F486V reversion using structural comparisons. BA.5/BA.4 and BA.2.12.1 relative to BA.2 showed higher neutralization evasion from the plasma of three-dose vaccinees and, most notably, from vaccinated BA.1 convalescents.

Nevertheless, due to the high sensitivity to E484, N440, N501, and K417 mutations, BA.1 infection could also generate new clones of BA.1-specific antibodies that could neutralize BA.1 yet not the SARS-CoV-2 WT strain. Interestingly, because of F486V and D405N, these NAbs were mostly evaded by BA.2 subvariants and BA.5/BA.4, which have narrow neutralization breadths. In terms of therapeutic NAbs, CoV2-2130 (Cilgavimab) and LY-CoV1404 (Bebtelovimab) could still neutralize BA.5/BA.4 and BA.2.12.1, albeit the R408S, S371F, and D405N mutations harbored by the BA.5/BA.2/BA.4 sublineages would render most expansive sarbecovirus NAbs useless.

These data imply that various recent Omicron sublineages could develop mutations to avoid humoral immunity induced by the Omicron BA.1 subvariant infection. Omicron's continued advancement presents significant hurdles to SARS-CoV-2 herd immunity, suggesting that BA.1-derived COVID-19 vaccine boosters might not be the best option for obtaining broad protection.


Letting this virus spread widely through the global population is accelerating viral evasion and the degradation of vaccine+infection derived immunity. It also makes variant targeting boosters even more futile, as it appears that the most recent dominant variant(s) such as BA4/BA5 often specifically evolved to evade antibodies elicited against previous variants including the one presumably targeted by whatever updated vaccines. Not really hopeful about updated mRNA vaccines this fall given this.

It is low-key impressive that Bebtelovimab still works against every variant, but this is probably in large part due to limited availability. Not enough people have been treated with it so far to exert any real selection pressure, and IIRC there are known mutations that would decrease it's effectiveness that would like become selected if everyone could get it.

also, there needs to be a survey why the booster uptake is so slow... one would think, if they thought the jabs were necessary, the booster would be equally important. did people lose faith, did they succumb mentally to the anti vaxers propaganda?
need to know why, then they can target the undervaxed people appropriately

most younger ppl including myself probably feel that as we are lower risk the primary doses would be enough. me i would still consider it later especially if data shows protection from severe covid wanes considerably - i know protection against infection wanes - actually it starts off pretty low against omicron.

or if another strain of covid does what no other strain has done and wiped out protection from severe covid from 2 doses. but this will be like 2020 all over again and we may need omicron specific ones by then.
