I've encountered this same issue, I've tried all the above and sent from multiple IPs and different accounts. I believe the issue may stem from the recipient's server (as I only have this issue sending email to certain companies). Strangely enough the only workaround I've discovered is that I can reply to their messages though... So I recycle an old email in my inbox change subject etc. and it works, for me leastways *shruggs*
Bear in mind that's my personal experience, you can try the suggestions above and hopefully It'll work for you.
