Question about changing your last name for drivers license

I know someone who has divorced her Husband and has now changed back her last name to the original name she had before. I eh know she say she eh want to have nothing to do with the previous husband so she change it back.

So she get she ID Card already but she just want to know what documents she need to walk with when going licensing office in town because is a long way and she don't want to have to go back twice.

And also possibly how much it go cost.
