Pfizer COVID-19 vaccine approval expected early September

Last month, Pfizer announced that the FDA granted Priority Review designation for their COVID-19 vaccine Biologics License Application (BLA) — the application to fully approve the vaccine. This means that the application is prioritized, shortening the FDA’s review time from 10 months to 6 months.

At the time of the announcement, the FDA’s decision wasn’t expected until January 2022. However, it was recently reported that the FDA is now targeting approval in early September — with an unofficial deadline of September 6.

As of yet, the FDA’s Vaccines and Related Biological Products Advisory Committee (VRBPAC) hasn’t posted a meeting date for when they’ll be discussing Pfizer’s application.

What does full FDA approval of a vaccine do if it’s already authorized for emergency use?

It is important to note that the number of people who participated in the initial COVID-19 safety studies was similar to that in the safety studies of other commonly used vaccines, including vaccines for tetanus, diphtheria, whooping cough and meningitis. Over 43,000 adults participated in the early phases of the Pfizer BioNTech clinical trial, over 30,400 in Moderna’s and over 44,000 in Johnson and Johnson’s. Half the participants in each study were given a vaccine, while the other half were given a placebo.

Where EUA and full approval differ
From this point on, emergency use authorization and full FDA approval for COVID-19 vaccines follow different clinical study requirements.

For the emergency use authorization, the FDA requires at least half of the participants of the original studies to be followed for at least two months post-vaccination. This is because the vast majority of vaccine-related side effects occur right after vaccination.

Full FDA approval, on the other hand, requires participants in the original studies to be followed for at least six months. Reviewers look at data from the same study participants but collected over a longer period of time. All adverse events are examined. The manufacturer must also provide more detailed manufacturing plans and processes, as well as a higher level of oversight and inspections. All of this adds significantly more time to the review process.

Both Pfizer and Moderna began their rolling submission for approval in the FDA’s “Fast Track” process, designed to speed up review. This allows the companies to submit portions of their approval application to the FDA for review as they’re completed.

So how long does the immunity from the vaccine last? Is this full approval for two shots or two shots and a booster as they are doing now?

In a major stride towards producing Covid-19 vaccine in the country, the government has struck a trilateral agreement for bottling, labelling and dispensing China's Sinopharm vaccine.

Under the memorandum of understanding (MoU) signed between the government, China's Sinopharm and Bangladesh's Incepta Vaccines Ltd yesterday, the local vaccine producer will supply five million doses of Covid vaccine a month from its plant in Savar.

No problem here, it 2 "safe" vaccines. This type of thing creates doubts.

Teen who had heart attack after vaccine dose to receive $225,000

SINGAPORE — The 16-year-old teenager who suffered a heart attack six days after receiving his first dose of the Pfizer-BioNTech/Comirnaty COVID-19 vaccine will be eligible for a one-time financial assistance of $225,000.

This will be provided under Singapore's Vaccine Injury Financial Assistance Programme (VIFAP), said the Ministry of Health (MOH) on Monday (16 August) in a press statement.

It added that medical investigations have found that the youth had developed acute severe myocarditis which led to the out-of-hospital cardiac arrest.

"The myocarditis was likely a serious adverse event arising from the COVID-19 vaccine he received, which might have been aggravated by his strenuous lifting of weights and his high consumption of caffeine through energy drinks and supplements," it said.

The teenager is currently "recovering steadily" and undergoing inpatient rehabilitation, the MOH added.

BANGKOK, Aug 17 (Reuters) - Thailand will purchase an additional 12 million doses of Sinovac's (SVA.O) coronavirus vaccine to try to expedite its rollout, a senior official said on Tuesday, despite concerns over its efficacy against the Delta variant of COVID-19.

Thailand's strategy had planned to use mainly locally produced AstraZeneca (AZN.L) vaccines but with supply short of the government's target, Sinovac shots are being used in a mix-and-match approach to inoculate the population faster.

The mass vaccination campaign started in June but the rollout is being hastened by Thailand's worst coronavirus wave so far, which is challenging Bangkok's healthcare system at a time when just 7.3% of the population has been fully vaccinated.

Sinovac's inactivated virus vaccine will be used as a first dose and AstraZeneca's viral vector vaccine the second, a mix that officials say protects against the Delta variant.

Thailand was the first country to announce it would use this combination.

"A first shot of Sinovac follow by AstraZeneca significantly boosts immunity," said Chawetsan Namwat, a senior health official at the Department of Disease Control.

COVID-19 fallout:

Moderna set to start human trials of experimental mRNA HIV vaccine

Moderna could start trials of its experimental mRNA HIV vaccine as early as 19 August, according to a new post from the firm on clinicaltrials.gov.

Moderna is looking to enrol 56 healthy adults without HIV to test the safety of the vaccine mRNA-1644 and basic immune response.

The vaccine is a collaboration between Moderna, the International AIDS Vaccine Initiative (IAVI) and the Bill and Melinda Gates Foundation (BMGF).

The candidate, mRNA-1644, is based on the same mRNA platform as Moderna’s highly effective Covid-19 jab, which is one of two of the only mRNA vaccines to be authorised anywhere in the world. The other is Pfizer-BioNTech’s Covid-19 vaccine.

The one-shot J&J vaccine has arrived: 108,000 doses

The Minister of Foreign and CARICOM Affairs is pleased to inform the national community of the scheduled arrival of 108,000 WHO-approved Johnson & Johnson COVID-19 vaccines, from the African Medical Supplies Platform (AMSP), at the Piarco International Airport at 7 p.m. on Thursday.

This shipment of vaccines is the first tranche out of an order of 800,000 made by Trinidad and Tobago through the AMSP, with further tranches to follow as additional supplies become available.

The delivery of these Johnson & Johnson single dose vaccines to Trinidad and Tobago at this time is the result of intensive dialogue and diplomacy, led by Dr Keith Rowley during his Chairmanship of CARICOM with His Excellency Cyril Ramaphosa, President of the Republic of South Africa; His Excellency Nana Akufo-Addo, President of the Republic of Ghana; and His Excellency Uhuru Kenyatta, President of the Republic of Kenya.

These discussions, with follow-up by the CARICOM Secretariat, led to the inclusion of Trinidad and Tobago and several other CARICOM Member States on the AMSP, through which Trinidad and Tobago and the region are accessing a significant supply of COVID-19 vaccines that would otherwise not be available to us on the open market.

COVID-19 vaccines may trigger superimmunity in people who had SARS long ago

By Jon Cohen Aug. 18, 2021 , 5:15 PM

Almost 20 years before SARS-CoV-2, a related and even more lethal coronavirus sowed panic, killing nearly 10% of the 8000 people who became infected. But the 2003 outbreak of severe acute respiratory syndrome (SARS) may have left some survivors with a gift. Former SARS patients who have been vaccinated against COVID-19 appear able to fend off all variants of SARS-CoV-2 in circulation, as well as ones that may soon emerge, a new study suggests. Their formidable antibodies may even protect against coronaviruses in other species that have yet to make the jump into humans—and may hold clues to how to make a so-called pancoronavirus vaccine that could forestall future outbreaks.

What the new vaccine data does and doesn't tell us

The new coronavirus vaccine data released yesterday by the CDC confirms what other recent research has been saying: The coronavirus vaccines' effectiveness against infection has decreased over time.

Between the lines: There's little to no data that the vaccines' effectiveness against hospitalization will eventually follow suit.

But the fact that it could is the Biden administration's main justification for a massive booster campaign — a risk-averse approach that's being criticized as unethical by some experts.

Driving the news: The CDC released three new studies focusing on the vaccines' effectiveness, particularly in light of the Delta variant.

One looked at Pfizer and Moderna's effectiveness against infections among nursing home residents over time, and found that it dropped from 75% pre-Delta to 53% when Delta became dominant. It didn't differentiate between asymptomatic, symptomatic and severe infections.

Another used data from 21 hospitals to estimate the mRNA vaccines' effectiveness against hospitalization over time, and found there was no significant change in effectiveness from mid-March to mid-July.

The third, using New York state data, found that all three vaccines' effectiveness against infection dropped from 92% in early May to 80% at the end of July, but the effectiveness against hospitalization remained relatively stable.

Reality check: This is all good news for most vaccinated people — your vaccines will keep you alive and out of the hospital.

It's not so great for some vulnerable populations, particularly nursing home residents, who may be less protected than they'd thought.
