WASHINGTON, Oct. 8, 2014 – The potential spread of Ebola into Central and Southern America is a real possibility, the commander of U.S. Southern Command told an audience at the National Defense University here yesterday.

“By the end of the year, there’s supposed to be 1.4 million people infected with Ebola and 62 percent of them dying, according to the [Centers for Disease Control and Prevention],” Marine Corps Gen. John F. Kelly said. “That’s horrific. And there is no way we can keep Ebola [contained] in West Africa.”

If it comes to the Western Hemisphere, many countries have little ability to deal with an outbreak of the disease, the general said.
“So, much like West Africa, it will rage for a period of time,” Kelly said.

This is a particularly possible scenario if the disease gets to Haiti or Central America, he said. If the disease gets to countries like Guatemala, Honduras or El Salvador, it will cause a panic and people will flee the region, the general said.

“If it breaks out, it’s literally, ‘Katie bar the door,’ and there will be mass migration into the United States,” Kelly said. “They will run away from Ebola, or if they suspect they are infected, they will try to get to the United States for treatment.”

Also, transnational criminal networks smuggle people and those people can be carrying Ebola, the general said. Kelly spoke of visiting the border of Costa Rica and Nicaragua with U.S. embassy personnel. At that time, a group of men “were waiting in line to pass into Nicaragua and then on their way north,” he recalled.

“The embassy person walked over and asked who they were and they told him they were from Liberia and they had been on the road about a week,” Kelly continued. “They met up with the network in Trinidad and now they were on their way to the United States -- illegally, of course.”
Those men, he said, “could have made it to New York City and still be within the incubation period for Ebola.”


Kelly said his command is in close contact with U.S. Africa Command to see what works and what does not as it prepares for a possible outbreak in the area of operations.

The Ebola outbreak in Nigeria appears to be nearing a possible end due to a rapid response coordinated by Nigeria’s Emergency Operations Center with assistance from international partners, including the Centers for Disease Control and Prevention (CDC). The official end to an Ebola outbreak comes when two of the 21-day incubation periods for Ebola virus have elapsed without any new cases.

During the outbreak, there were 19 laboratory-confirmed and one probable Ebola cases in two Nigerian states. Nearly 900 patient contacts were identified and followed; all but three have completed 21 days of follow-up period without Ebola symptoms. There have been no new cases since Aug. 31 and the last three patient contacts will exit their 21-day follow-up on Oct. 2 – strongly suggesting the outbreak in Nigeria has been contained. A report on Nigeria’s response to the outbreak appears in a Sept. 30 early release issue of CDC’s Morbidity and Mortality Weekly Report (MMWR).

"Although Nigeria isn’t completely out of the woods, their extensive response to a single case of Ebola shows that control is possible with rapid, focused interventions,” says CDC director Tom Frieden, MD, MPH. “Countries throughout the region as well as Nigeria need to take rapid steps to prepare for possible cases of Ebola in order to prevent outbreaks in their country.”

On July 20, 2014, a very sick traveler from Liberia arrived at the international airport in Lagos, Nigeria, a densely populated city of 21 million and a regional hub for economic, political, and travel. By the time it was recognized that the patient carried the Ebola virus, he had exposed 72 people on commercial aircraft, at an airport, and at a hospital. It appeared that conditions were ripe for the disease to spread through the city, the nation, and perhaps to other parts of Africa and beyond.

Nigerian authorities took several quick actions. Nigeria’s Port Health Services began contact tracing at the airport and worked with airlines and partners to ensure outbreak notifications were made according to International Health Regulations established by the World Health Organization. Nigeria’s Ministry of Health activated an Ebola Incident Management Center (a precursor to its current Emergency Operations Center) and charged it with leading the national response; that center was led by an incident manager chosen for experience and competency.

Nigeria established its incident management system in October 2012 as part of its emergency plan for the global polio eradication initiative. Overall design of the Ebola response came from a senior strategy team that included international partners, including Doctors Without Borders, UNICEF, CDC, and WHO. Among its activities, the team developed a staffing plan that executed a social mobilization strategy that reached more than 26,000 households of people living around the contacts of Ebola patients.

Nigeria’s decision to use the emergency operations center to respond to the Ebola outbreak resulted in a rapid, effective and coordinated response, according to the MMWR.

Due to national preparedness and prompt cross-border communication, Senegal avoided an Ebola epidemic after confirming its first case of the disease on Aug. 29, 2014.

On Aug. 14, a man traveled in a seven-person taxi from Guinea to the densely populated city of Dakar, Senegal to visit his family. He fell ill two days later with fever, diarrhea, and vomiting and sought care at a neighborhood clinic, where he was treated as an outpatient – receiving intravenous fluids and other symptomatic care until he was admitted to a Dakar hospital on Aug. 26.

The patient did not tell anyone that his brother had died in Guinea on Aug. 10, and that he had helped his family prepare the body for burial. The same day that the patient entered hospital in Dakar, his mother and sister were admitted to an Ebola Treatment Unit in Guinea, they named the patient as a contact. Public health staff contacted the Senegal hospital on Aug. 27; he was promptly isolated and a blood sample was taken. On Aug. 29, testing by Pasteur Institute Dakar confirmed that the man had Ebola virus disease.

The Senegal Ministry of Health had been preparing for just such a situation. Health care staff was already trained about Ebola virus and infection control. An oversight committee organized laboratory testing, case investigation, and contact tracing.

Investigation quickly identified 67 contacts of the Ebola patient – 34 residents of the home where he stayed, and 33 healthcare workers. All contacts underwent a 21-day monitoring period beginning on Aug. 29; contacts were asked to submit to voluntary in-home quarantine and twice-daily temperature checks by Red Cross volunteers. Food was provided for all household contacts. On day one of monitoring, more than half of contacts were seen; by day five more than 90 percent of contacts had been seen.

By Sept. 18 all 67 contact had completed the 21-day follow-up. There were no further confirmed cases of Ebola.

Prompt notification of health officials in Senegal about the case by health officials in Guinea, and early preparations by the Ministry of Health and partners in Senegal for anticipated imported cases of Ebola resulted in a rapid containment response, cording to the MMWR. The report also found that it is important for neighboring countries to anticipate imported cases and define success as containment rather than exclusion of imported Ebola cases.

Canadian Ebola Vaccine Begins (Human) Testing

Human testing of an experimental Canadian-made Ebola vaccine began Monday, with federal officials saying the drug could be shipped to West Africa within months if it proves successful.

Health Minister Rona Ambrose said the launch of the vaccine's first clinical trial marks a promising step in the global campaign to contain the virus, which the World Health Organization says has killed more than 4,000 people, mostly in the West African countries of Liberia, Sierra Leone and Guinea.

"This provides hope because if the Canadian vaccine is shown to be safe and effective, it will stop this devastating outbreak," Ambrose said in a conference call from Calgary.

Twenty vials of the vaccine have been sent to the Walter Reed Army Institute of Research in Maryland for testing on about 40 healthy volunteers, she said.

The Phase 1 trial will determine if the vaccine created by Public Health Agency of Canada and known as VSV-EBOV is safe for human use. It will also determine the proper dosage level and test for possible side effects, Ambrose said.

Studies have shown the vaccine works in primates both to prevent infection when given before exposure and to increase survival chances when given quickly after exposure.

Canada's chief public health officer said results from the human trial are expected by December, and if successful, the next stage would be to test it in a larger human sample, including those directly handling Ebola cases in West Africa.

"The health-care workers on the ground are the most likely target to do the next step," which could begin by the end of the year or early 2015, Dr. Gregory Taylor said in a news conference in Toronto.

A small U.S. company called NewLink Genetics holds the licence for the vaccine and will be arranging the trials at the U.S. military lab.

NewLink said earlier this month that at least five clinical trials involving the vaccine would soon be underway in the United States, Germany, Switzerland and in an unnamed African country which is not battling Ebola. The Canadian government has also said it wants to conduct a trial in this country.

The aim of these early trials is to see if the vaccine is safe for human use and how much vaccine is needed to generate what is hoped to be a protective response in people.

I just heard they have a ebola patient in quarantine and the minister of health claims they could handle it...so they not alerting the public...

The people need info!

There was an Ebola virus scare at Piarco International Airport yesterday after London authorities flagged a Nigerian man, who was travelling aboard a British Airways (BA) flight to this country, as a possible case.
Speaking to the Express last night, acting Chief Medical Officer (CEO) at the Health Ministry Dr Colin Furlonge said this country’s health authorities im­mediately responded and were ready at the airport to swiftly deal with the situation.
However, he said the man —Sabastina Onwubgeuzie—was examined by a senior doctor at the ministry and cleared.
Flight BA 2159 originated in London Gatwick, England, then went on to St Lucia before landing in Trinidad before 4 p.m.
Authorities at the airport were alerted of the possible Ebola case via the Joint Regional Communications Centre, based in Barbados, via the Advance Passenger Information database.
According to their website, the database contains a complete list of persons (crew and passengers) on board, inclu­ding their biographic data and carrier information.
Passengers on board the flight were not allowed to disembark the plane while the Nigerian was taken into isolation and examined.
Furlonge said the man, who is married to a Trinidadian woman, poses no risk.
He said the man was born in Nige­ria and worked at the Port of Spain General hospital up to four years ago.
He then migrated to Australia where he has been for the past three years.
Furlonge said the man and his wife came to Trinidad for a three-week holiday.
The man was apparently flagged by authorities in London because he was born in Nigeria.
“Because they flagged it, we had to take precautions; we are on high-surveillance monitoring and assessing our nation’s airports and other ports of entry,” said Furlonge.
Furlonge said yesterday he met with members of the Defence Force to strategise how to work together to deal with any possible cases of the Ebola virus at this country’s ports.
He said further, a meeting is scheduled to take place next Tuesday between officials at the Health Ministry, National Security and Tourism, as well as other stakeholders involved, to develop a solid strategy in addressing any instances of the virus in this country.
“Because of the nature of the illness, the chances are slim of it reaching this region, but regardless, we must taken precautions,” said Furlonge.
Speaking with the Express yesterday evening, Health Minister Dr Fuad Khan, who is currently overseas, said he is aware of the scare and as a result spoke with National Security Minister Gary Griffith about having mechanisms in place for Immigration officers.
Not wanting to say much, Khan said he received news about the incident and is awaiting a report from health personnel.
Earlier this week, in an interview with reporters, Khan said Trinidad and Tobago was well prepared for the Ebola virus.•

Over last 36 hours the news generally indicate a more robust series of international interventions are being executed.
Things need to heat up to break the momentum of this aggressive virus since the probability of hopping international borders is also getting much higher.

Well the 'cats amongst the pigeons now' and every country globally has to prepare for this unwelcomed visitor.
BTW, I remember a few years ago hearing about illegals coming from Africa on our East Coast....hope this was just people fabricating stuff.

I was hearing is 2 suspected cases, but two different stories.

Some saying mt hope, some say caura.

Just heard Fuad on the radio talking about giving the go ahead for the purchasing of handheld thermometers, security corridors that scan people's temperatures while they walking through and protective suits...so I guess if according to him we were ready before all this now we must be super ready. FortressTnT.

of course the thermometers will not help unless the person is showing symptoms-but i guess nothing else can be done.
