WTK: PLEA Passport

I looking to acquire a PLEA Passport but I only saw one place in south that offering this advertised in the newspaper. Does anyone know any other places preferably along the East-West corridor? Any info would be much appreciated...contact numbers, location, time duration, quality, cost.etc..thanks

^^ I checked the thread and saw 2 places mentioned, HHSL in Pt. Lisas and one in Macoya. Anywhere else?
