Can someone assist me in filling out this form its my first time

I imported a nissan caravan i need to get it licence now
i need help with number (3), (4), (7) and (8) in the form
where can i get the weight from licence office of the export cert?
and what documents are needed and how many copies and what order must they be in?

7) Take out a 1 day insurance at Bankers Insurance next door to licensing office and use the info on that to fill out number 7.

The english Export certificate from japan should have all the info needed for 3, 4 and 8.

And if you still don't know, the license inspectors will be there on the day to help make sure all the documents are correct before they inspect the car. Hope that helps.
