It turns out it was a bad fuel situation. TTTL ran diagnostics and gave me a quote to remove and flush rail and injectors. Haven't done it yet tho. Filled a fresh tank of fuel and no more problems. If is glass alone you can get that cut and replaced easily. For the whole assembly brand new, check 4x4 Trinidad. Had to change mine recently, unfortunately I went to Toyota when there prices were close to 40% less at 4x4 Trinidad.
