WTK Licensing Appointments

Anyone can advise me if the 'Application form 4' is. Provided at license office to be filled out or do I have to download it? I can't seem to get the file downloaded.

Any help on the topic would be much appreciated

Forms are to downloaded
They are the types of PDF you can type into, so all you need to do is fill out, print and sign.
Print it double sided
They fuss if its 2 pages.

I'm getting it when I google search it, but for some reason it's not opening or downloading to my device.

Anybody know if lisencing office still honoring appointments between now and the 23rd? Been trying to get in contact to find out and nothing. I have an appointment on the 10th.

they called and cancelled mine

Know someone who had an appointment today at 8:30am and they called yesterday and cancelled and that they'd reschedule.

They cancelled a few appointments from what i heard

I actually got an email response from MOWT on tuesday when I queried and the email stated if I did t get a phone call, the appointment should be on. I guess I'll see on Monday, appointment is at the Guaico office. Gonna suck to drive all the way up there to get blank tho.
